• The inspector found employee reporting agreements were not available in the mobile unit.
• There were no written procedures for clean-up of vomit and diarrhea.
• Raw shrimp was stored over ready-to-drink Horchata in the refrigerator.
• Ingredients were not kept cold enough. In the cooler were cut lettuce at 65 degrees; cut tomato, 58; salsa, 54; and shredded cheese, 56. In the refrigerator, there was cooked meat, 62 and shredded cheese, 52. Iowa code requires food be kept at 41 degrees except during preparation. The foods were thrown away.
• The paper towel dispenser at the sink was empty. Staff then provided napkins for the purpose of hand drying.
